1. Register with USAV Florida Region
($45.00-if you don’t make a team, the region will reimburse you the fee)

2. Register with AAU ($12.00)

3. Medical Release Form

Bring this form with Notary & signatures to tryout or preregistration.

4. Bring a copy of your child’s birth certificate

5. Bring tryout fee $30.00 (cash or check made out to FMVBA)
